Practice submitted to ORU Fogar’s II Regional Best Practice Award by the Basque Government (Spain), on the 28th of April 2017, with entry register number 12 and a total score of 516 points.
This good practice consists in the development of a new organisational model for integrated care for patients with complex needs focused on improving health care through: 1) coordination and communication between health professionals; 2) provision of patient-centred care; and 3) use of ICT as a facilitator. Positive evaluation results confirm the satisfaction of all the agents analysed; patients and caregivers feel safer and more controlled, and healthcare professionals validate their way of caring for these patients. Through the prevention and promotion of health and patient empowerment, the new model favours the transition from specialized care to primary care. Predictive analysis to estimate resource use in the medium and long term has confirmed that the intervention is cost effective.
